# Fill in a form from an organization

> Completing a form attached to a donation or sent to you by an organization.

Some appeals ask for a short form as part of giving — before you pay, right after, or sent to you separately by the organization.

## Before you start

This only applies if the organization you're giving to has attached a form — most donations don't have one.

## Steps

1. **If the form appears before payment**, it's shown to you as a step inside checkout, and you'll need to complete it before your donation can go through.

2. **If the form is offered after payment**, you'll see "Donation successful 🎉 One more step — please complete a short form for the organizer." on the confirmation screen.

3. **Select Complete the form** to fill it in (it opens in a new tab), or select **View Receipt** if you'd rather do it later — a reminder email with the form's link usually follows after a few days if you skip it.

4. **You might also receive a form link directly from the organization**, with no donation attached to it at all — for example a volunteer sign-up or a membership application. Open the link the same way.

5. **Fill in the fields and select Submit.** Confirmation: a thank-you message appears once your response is recorded.

## If something goes wrong

| What you see | What it means | What to do |
|---|---|---|
| "Form not found." | The link is old, or the organization has removed or renamed the form | Ask the organization for a current link |
| "Prefill link is invalid or expired." | A personalized link from an email has timed out | Ask the organization to resend it, or open the form from scratch and fill it in yourself |
| "Please complete the captcha before submitting." | The human-verification box wasn't ticked | Tick the box, then select Submit again |
