Fill in a form from an organisation
Some appeals ask for a short form as part of giving — before you pay, right after, or sent to you separately by the organisation.
Before you start
Section titled “Before you start”This only applies if the organisation you’re giving to has attached a form — most donations don’t have one.

If the form appears before payment, it’s shown to you as a step inside checkout, and you’ll need to complete it before your donation can go through.

If the form is offered after payment, you’ll see “Donation successful 🎉 One more step — please complete a short form for the organizer.” on the confirmation screen.

Select Complete the form to fill it in (it opens in a new tab), or select View Receipt if you’d rather do it later — a reminder email with the form’s link usually follows after a few days if you skip it.

You might also receive a form link directly from the organisation, with no donation attached to it at all — for example a volunteer sign-up or a membership application. Open the link the same way.

Fill in the fields and select Submit. Confirmation: a thank-you message appears once your response is recorded.
If something goes wrong
